SUPPORTED LIVING WITHIN A CLUSTER

UCP offers supported living within a cluster. This means individuals can live in apartment settings, on their own, or with a roommate of their choice. Staff support is provided to teach basic skills, banking, budgeting, and shopping. Health management is designed to allow a person to live in their own home successfully. Individuals may be provided transportation and accompanied into the community but also may travel independently on public transportation to and from work or day activities. Rent subsidies are made possible through DDS. The typical use of this service is 5-20 hours a week. This service provides intermittent support and is less than 24-hour-a-day support. In our cluster setting, overnight support is provided from 11 pm to 7 am daily.

IN-HOME SUPPORTS

This service is designed to allow a person to live in their family home while UCP staff teaches and provides assistance for basic skills, banking, budgeting, shopping and community access.  The typical use of this service is 5-20 hours a week. This service provides intermittent support and is less than 24-hour a day support.

INDEPENDENT LIVING

Individuals live in settings of their choosing and are empowered to support themselves by paying their own rent and bills and traveling independently to their place of employment or in their daily life. Staffing supports are provided intermittently for healthcare management, banking, budgeting, shopping, and community access. Rent subsidies are made possible through DDS.  The typical use of this service is 5-20 hours a week. This service provides intermittent support and is less than 24-hour-a-day support.

COMING SOON: REMOTE SUPPORTS

Remote Support enables a person to gain more independence with less reliance on staff to be physically present to receive support. The remote caregiver can interact, coordinate supports, monitor, and/or respond to the person’s needs through technology equipment capable of live two-way communication. Remote supports include both a service and a technology component.
