UCP’s Transition program aims to help students with a disability leave school with a paid job and a seamless transition to adult life and support services.

School to Work takes a team approach to offering direct services to students. Students work with a Transition Specialist who will partner with the student’s team of educators and collaborate with parents and the adult service system to help the student find a good job match before they exit school. 

School to Work offers training and resource coordination for students and families enrolled in the program.

  • Job seeking skills
  • Job coaching services
  • Placement assistance and follow up
